Care Coordination and Advocacy
Qualified professionals manage client intake and annual assessments to provide individual care plans. Case managers assist clients in navigating the care system when they face crisis situations.
Resources and referral are available for those transitioning from work to disability. This program is made possible through funding from the Haag Foundation and the United Way of Lee County.
After registration, you will receive a comprehensive intake and evaluation which will determine your individual care plan and recommended services.
Presentations from experts in disability law, work/employee benefits, and insurance plans are offered to clients.
Local and National Advocacy
We track legislation and issues that affect our clients and notify them in time for them to benefit.

Multiple Sclerosis Center of Southwest Florida
The Multiple Sclerosis Center of Southwest Florida is a 501(c)3 nonprofit organization that has served the community since 2000. Its mission is to improve the quality of life for those in Southwest Florida affected by multiple sclerosis. Services include specialized exercise classes, care coordination, art therapy, mental health counseling, support groups, connection to resources, and educational activities.
The MS Center is not a chapter of a national organization. All donations received stay here in Southwest Florida to serve our local community.
